LIGHTHOUSE SPILL REPORTS

Prior to cleaning up any type of spill, you must first open a Spill Report and take a photo. Upon completion of cleaning the spill, you will finish the form by answering the questions and taking another photo.

Many of the customer incidents that occur are due to a spill or after a spill has been cleaned up. The before and after pictures are to document the time the spill was cleaned and to capture the Caution (or Wet Floor) Sign is present.

You have two options when filling out a Spill report, the short way and the detailed way. Below are step by step instructions on how to complete a Spill Report in Lighthouse using both methods.

 

HOW TO COMPLETE A SPILL REPORT (short version)

Open Lighthouse and login

Click the blue circle on bottom right corner and select Task

In the top search bar type Spill Report and choose it from the dropdown menu.

Enter your location

**All questions in this report are not mandatory and can be skipped. The only mandatory step is the before and after photo.**

Type the area of the store the spill was found

Take a Photo of the spill before it is cleaned

Take a photo of the area once spill is cleaned

HOW TO COMPLETE A SPILL REPORT (detailed version)

Open Lighthouse and login

Click the blue circle on bottom right corner and select Task.

In the top search bar type Spill Report and choose it from the dropdown menu.

Enter your location

Type the area of the store the spill was found

Walmart Spill Notification -To capture details of liquid spills or products on the floor that could have potential of causing a slip and fall. Capture area, type of spill and take photo prior to and after cleaning up spill. If spill is caused by leak, please complete a Safety and Maintenance Report.

Take a Photo of spoiled area upon arrival (Include the Caution/Wet Floor Sign)

Select the type of soil that you will be cleaning (check all that apply)

  • Leaking Freezer
  • Liquid Spill (i.e. juice)
  • Biohazard concern (i.e. vomit, blood)
  • Broken glass
  • Sharps (needles)
  • Dry spill
  • Excess water at entrance due to weather
  • Leaking roof
  • Leaking pipe or fixture (i.e. toilet)

Did you set up a hazard cone upon arrival to clean area?

  • Yes
  • No

Were you called by Walmart to clean the spill?

  • Yes
  • No

Did anyone slip/fall?

  • Yes
  • No

If Yes, select one of the following

  • Witnessed slip and fall
  • Told of slip and fall

After cleaning, is the area clean and free of hazards?

  • Yes
  • No

Take a photo of the area after the spill has been cleaned (include the Caution/Wet Floor Sign)
