HOW TO COMPLETE: LIGHTHOUSE ENTRANCE TASK FORM

  • Log into Lighthouse via your Smartphone
  • Click the blue circle at the bottom right of the page
  • Tap the Tasks option from menu
  • Search for Walmart Entrance Porter Tasks, select it from the bottom list
  • Tap on Location line and search for the Walmart location you are in
  • Tap this line and select the Entrance in which you are reporting tasks for

CONDITION UPON ARRIVAL

Step 1 – Are clean, dry, free of litter, debris and hazards? Choose one answer

  • Yes
  • No

If No, indicate condition in box below (Maintenance/Safety issues are to be reported to store manager)

Step 2 – Submit a photo of area upon arrival

ENTRANCE TASKS

Please enter all tasks performed

  • If the tasks below were completed, please check the box next to it:
  • Floor sweeping
  • Microfiber mop cleaning (if the answer is no, you indicated that the floor was dry when arriving at the port of this area)?
  • Check door glass and clean stains, if necessary?
  • Disinfect contact points?
  • Check and empty trash can, if necessary?

CONDITION UPON COMPLETION

General questions regarding the state of the entrance after doorman functions have been performed
